Nurses educate patients and caregivers on a variety of information, including but not limited to, medication safety, treatment and management of symptoms, education on their illness/condition, and monitoring of the patient’s care to ensure his/her health is stabilized. Nurses develop individualized care plans for each patient based off of the primary physician’s orders as well as their initial assessment with the patient. All nurses work closely with the patient’s primary care physician to ensure the utmost quality of care for each patient. Our nurses maintain an open line of communication between the patient, his/her caregivers and his/her primary care physician.
In the event that a patient does not have a primary care physician, Helping Hand can assist the patient in finding a visiting physician to the patient’s home.
